29 CFR 1910.28(b)(3)(ii): (ii) Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a. On or about February 28, 2018, located in front of autoclave #1, there was a 16 inch deep floor hole that was approximately 71 inches wide by 43 inches long, thereby exposing the employees to a slip trip fall hazard of 16 inches. b. On or about February 28, 2018, located in front of autoclave #12, there was a 20 inch deep floor hole that was approximately 77 inches wide by 44 inches long, thereby exposing the employees to a slip trip fall hazard of 20 inches. c. On or about February 28, 2018, located in front of autoclave #2, there was a 20 inch deep floor hole that was approximately 69 inches wide by 57 inches long, thereby exposing the employees to a slip trip fall hazard of 20 inches. d. On or about February 28, 2018, located in front of autoclave #11, there was a 15 inch deep floor hole that was approximately 63 inches wide by 39 inches long, thereby exposing the employees to a slip trip fall hazard of 15 inches. e. On or about February 28, 2018, located in front of autoclave #5, there were two 15 inch deep floor holes created with the ramp up with one approximately 6 inches wide by 39 inches long and the other hole being 30 inches wide by 39 inches long, thereby exposing the employees to a slip trip fall hazard of 15 inches. f. On or about February 28, 2018, located in front of autoclave #9, there was a 10 inch deep floor hole that was approximately 65 inches wide by 58 inches long, thereby exposing the employees to a slip trip fall hazard of 10 inches. g. On or about February 28, 2018, located in front of autoclave #6, there was a 10 inch deep floor hole that was approximately 63 inches wide by 58 inches long, thereby exposing the employees to a slip trip fall hazard of 10 inches. h. On or about February 28, 2018, located in front of autoclave #8 there was a 15 inch deep floor hole that was approximately 63 inches wide by 58 inches long, thereby exposing the employees to a slip trip fall hazard of 15 inches. i. On or about February 28, 2018, located in front of autoclave #10, there was a 15 inch deep floor hole that was approximately 63 inches wide by 58 inches long, thereby exposing the employees to a slip trip fall hazard of 15 inches. j. On or about February 28, 2018, located in front of autoclaves #7, #3, and #13, there was an 18 inch deep floor hole that was approximately 23 feet 9 inches wide by 51 inches long with a 48 inch platform between #7 and #3 autoclaves, thereby exposing the employees to a slip trip fall hazard of 18 inches. k. On or about February 28, 2018, located in front of autoclaves #16, #15 and #14, there was a 17 inch deep floor hole that was approximately 23 feet 11 inches wide by 52 inches long with a 70-1/2 inch platform between #14 and #15 autoclaves, thereby exposing the employees to a slip, trip, fall hazard of 18 inches. l. On or about February 28, 2018, located in the Banbury mill there was a 16 inch deep floor hole that was 16 inches by 28 inches in size, thereby exposing employees to a trip, slip, fall hazard of 16 inches.
