29 CFR 1910.119(l)(1): The employer did not implement written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process. On or about 02/15/2018 and at times after, in Area 320 of the CITGO Refinery in Lake Charles, the employer did not implement its management of change process when it deviated from its incident investigation and near miss/deficiencies observed in Area 320 on February 15, 2018 with the following instances: a. Non-functional instrument air gages on the Light Naphtha valve (LV052) was not investigated and tracked in the employers incident tracking system (IMPACT). b. Broken Eaton, pump switch on the Fractionator Reboiler Pump (JP8) was not investigated and tracked in the employers incident tracking system (IMPACT). c. Open ended pipe on the seal pump system for the debutanizer reboiler pump (JP6A) was not investigated and tracked in the employers incident tracking system (IMPACT). d. Open ended pipe found above the Depentanizer reboiler (C108) associated with instrument number 320LSL083 was not investigated and tracked in the employers incident tracking system (IMPACT)
