29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): The employer is failing to train employees on how to operate the powered industrial trucks and recognize the struck by or caught under hazards associated with their operation. The most recent occurrence was evident during the inspection conducted on February 14, 2018 at the facility located at 1001 Gallup Dr, Omaha, NE 68102. The employer has not trained employees who operate powered industrial vehicles to move and stack loads of paper. Instances of inadequate training include but are not limited to: i. Employees operating powered industrial trucks without having received formal instruction on the safe handling operations as outlined in the operators manual and confirmed with a written test. ii. Employees operating powered industrial trucks without having received practical training (demonstrations by the trainer) and practical exercises (performed by the trainee) on the safe handling operations as outlined in the operators manual. iii. Employees operating powered industrial trucks without having been evaluated on their performance in the workplace.
