29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): (a) William E. Strait - During the period January 18, 2017 through January 19, 2018 deck openings around aft end of engines in the upper engine room were not guarded exposing employees to fall hazard of up to 8 feet.
