29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): Authorized and affected employees in all 3 buildings of the facility were exposed to electrical, struck-by and caught-between hazards associated with the unexpected energizing or start-up of the equipment in that written program for control of potentially hazardous energy on equipment had not been developed. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
