A) Zehr's Metal & Flat Roofing LLC on or about January 26, 2018 at the address of 7973 Quarry Road, Maumee, Ohio, 43537: The employer did not ensure employees were protected from falls while installing new roofing materials to a residential type structure. The employees were exposed to a potential 10 foot fall to the level below.
