29 CFR 1910.147(d)(6): The employer did not ensure that prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee verified that isolation and deenergization of the machine or equipment had been accomplished. This violation occurred on or about January 18, 2018; where employees in the maintenance department were required to perform service and maintenance work on machines and equipment without verifying deenergization of hazardous energy. Employees are exposed to electrical hazards.
