29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operators performance shall be conducted at least once every three years: The employer is failing to protect employees from struck-by hazards. This violation was most recently documented at a worksite located at or near 88818 543 Ave., Bloomfield, Nebraska, 68718 on January 18, 2018. The employer did not ensure that employees performances were being evaluated at least once every three years prior to operating a forklift. Abatement certification is required for this violation. Abatement certification sheet is enclosed with the citation.
