29 CFR 1910.269(d)(2)(iii): Procedures were not developed, documented, and used for the control of potentially hazardous energy covered by paragraph (d) of this section: Tesla, Inc., Hampshire College MV Metering Panel, Amherst, Massachusetts; On or about December 29, 2017, employees were exposed to 13,800 volts while entering an energized panel to take pictures of the current transformers without utilizing the hazardous energy control procedure for the equipment.
