29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: a) At the facility in the mixing area, where the floor vertical mixers were not equipped with guards over the bowls while mixing agitators were in a rotating motion. This condition exposed the employees to a caught-in and amputation hazard. b) At the facility in the production area, where the Benier B-9 moulder attached to the po'boy machine was not equipped with a guard over the rollers. This condition exposed the employees to a caught-in and amputation hazard. c) At the facility in the production area, where a tool was not used to push the dough down into the divider machine. This condition exposed employees to an amputation hazard. d) At the facility in the production area, where the Oddy Novatek 2 front panel access door was able to be opened while the machine was in operation. This condition exposed the employees to an amputation hazard.
