29 CFR 1928.57(c)(4)(i): Guards, shields, and access doors were not in place when the equipment is in operation: The employer is failing to protect employees who are performing clearing operations on mill equipment in the facility are protected from hazards including but not limited to amputations and crushing. This was most recently documented at 33409 749 Road. Imperial, Nebraska on or about December 2, 2017 when the employer had an employee clear the rocks from the screen of the mill machine and suffered an amputation. The employer is failing to ensure all guards are in place during operation.
