29 CFR 1910.119(d)(3)(i): The employer did not compile information pertaining to the equipment in the process, including but not limited to: A) Material of Construction: a) This violation was most recently observed on or about December 6, 2017 in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas, when the employer failed to compile the design specifications/materials of construction, the Manufacturer's Certificate of Compliance (Form U1 or U1-A) and/or as built engineering drawings for the Hydrogen Peroxide Day Tank T-1112. b) This violation was most recently observed on or about December 6, 2017 in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas, when the employer failed to compile the design specification/materials of construction, piping calculations, date of installation, manufacturer's data report (MDR) and the manufacturer's test report (MTR) for the following process piping: 12" BzCl-XX-0701, 2" BzCl-XX-0404, and 1/2" H2O2-XX-0405. D) Relief system design and design basis: This violation was most recently observed on or about December 6, 2017 in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas, when the employer failed to complete the Process Safety Information (PSI) for the design and design basis for the Reactor Vent Scrubber S-1101. F) Design Codes and Standard: This violation was most recently observed on or about December 6, 2017 in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas, when the employer failed to compile the design codes and standards for BPO process vessels such as Primary Reactor R--1101, Hydrogen Peroxide Day Tank T-1112, Benzoyl Chloride Storage Tank T-1103.
