29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) Mitsubishi Electrical Discharge Machine (Type DWC-200): On or about December 6, 2017, employees were exposed to an electrical shock hazard while working on and around the machine. The flexible metal conduit (FMC) was pulled away from the machine electrical connection fitting, exposing live electrical wires and electrical wiring were improperly spliced together.
