29 CFR 1910.23(c)(8) The employer did not ensure that the top cap and the top step of a step ladder were not used as steps. a) Maintenance department: On or about December 14, 2017, and at times prior to, employees used the last step of the ladder to reach into Peterson 6700B horizontal grinder, exposing them to falls. b) Line 3: On or about November 17, 2017, and at times prior to, employees used the last step of the step ladder to reach into the Amadas Open Mouth Bagger to remove product that had become stuck.
