29 CFR 1926.1053(b)(16): The employer does not ensure that portable ladders with structural defects, such as, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. This violation was most recently observed on 11/28/17, at a jobsite located at 31 Market Street, Ipswich, MA 01938, where employees using two portable ladders that were damaged and not taken out of service. Prior violations referred to include a violation of this Occupational Safety and Health Standard which is contained in: William Trahant, JR. Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(16), which was contained in OSHA inspection number 1003916, citation number 1, item number 3, and was affirmed as a final order on July 27, 2017, with respect to a workplace located at Congress Street, Salem, MA.
