Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1926.1204(c)(3): The employer did not isolate the permit space and physical hazard(s) within the space: On or about November 15, 2017, at the above addressed jobsite, employees were performing construction type work activities, to include permit required confined space entry activities (elevator pit), during which the employer did not isolate the permit space, nor the elevators energy (locked-out/tagged-out), and wherein an employee who was working in an elevator pit, was crushed by an elevator car and was subsequently seriously injured and hospitalized, and also resulted in exposing the other employees working at this job site to permit space physical hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
