29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) This violation occurred on or about November 17, 2017 when the employer did not ensure that three (3) employees required to operate powered industrial trucks were formally trained to do so; exposing employees to a struck-by hazard. (b) This violation occurred on or about November 17, 2017 when the employer did not ensure that four (4) employees required to operate powered pallet jacks were formally trained to do so; exposing employees to a struck-by hazard.
