29 CFR 1910.272(g)(4): The employer did not provide equipment for rescue operations, which was specifically suited for the bin, silo, or tank being entered: On or about October 19, 2017, at this at this grain handling facility, the employer failed to ensure rescue equipment was available on site during bin entry operations. This exposed employees to engulfment, asphyxiation, and/or amputation hazards.
