The employer did not have in writing evidence that such a survey had been performed: a) On or about September 22, 2017, at the Schlumberger J Plant, an engineering survey was not performed to determine the condition of the possibility of unplanned collapse of any portion of the structure prior to the start of demolition operations, exposing employees to fall hazards.
