29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation: a) Thought the facility - On or about October 17, 2017, and at times prior to, employees were not trained on the hazards associated with Sweating Slab Syndrome (condensation build up) prior to operating the forklifts, exposing employees to struck-by hazards.
