Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving. a) U-Haul Co. of Pennsylvania - On or about October 6, 2017, and times prior to, a Machine Operator was loading steel sheets from the Whitney machine to an employer modified scissor lift table that was not anchored to the floor. The lift table upended and came down on the operator with fatal injuries. These unapproved modifications to the scissor lift caused the scissor lift to up-end, lunge forward, and come down on the Machine Operator resulting in fatal crushing injuries. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
