29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices The employer is failing to protect employees from struck by and crushing hazards associated with servicing and maintaining equipment with stored energy. The most recent occurrence of this was evident on September 27, 2017 during the inspection of the facility located at 105 N. Main St. Wakefield, NE; where the employer has employees maintaining and servicing dock levelers without using the manufactured designed and installed maintenance prop as means of isolating the gravity and hydraulic crushing hazards which lead to the death of an employee on September 26, 2017.
