29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On July 16, 2017 located at the DAAV Maintenance Facility, an employee was working alone when mounting a 2075 pound sheet metal pan brake to the forks of a Hyster Forklift Model # H40XM, the load had not been secured and was not centered onto to the forklift causing a potential crushing hazard. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
