29 CFR 1910.269(l)(4)(ii)(A): The employer did not ensure that employees who use rubber gloves or rubber insulating gloves and sleeves as insulation from energized parts put on the rubber insulating gloves and sleeves in a position where he or she cannot reach into the minimum approach distance, established by the employer under paragraph (l)(3)(i) of this section: a) Jobsite: PECO Pole# 2802 - The employer did not ensure an employee used and put on rubber insulating gloves and sleeves where he could not reach into the minimum approach distance while repairing an energized 13.2kv downed primary line. Noted on or about 6/21/17.
