Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged. On or about June 10, 2017, a subcontractors employee was exposed to live electrical current of up to 27KV. The employee was contracted to drill three, 5 inch holes in the concrete floor of a large electrical cabinet located at 375 Victoria Road, Youngstown, Ohio 44515. The electrical current could be isolated and turned off using air switches on a corresponding electrical pole. The air switches were not locked out at the lockout point at the base of the pole.
