29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: a) Sawmill (Unscrambler and Stacker infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities. including but not limited to clearing lumber jams in and around the Unscrambler and Stacker infeed conveyor systems, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand. b) Sawmill (Trimsaw infeed and outfeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities, including but not limited to clearing lumber jams at the Trimsaw infeed and outfeed conveyor systems, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand. c) Sawmill (Sorter); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities, including but not limited to clearing lumber jams at the Sorter, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand. d) Sawmill (Log Deck, Bucksaw including infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities, including but not limited to unjamming wood debris, and cleaning in and around the Log Deck and Bucksaw infeed conveyor system, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand. e) Planer Mill (Stacker and infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities, including but not limited to cleaning and clearing jams at the Stacker and infeed conveyor systems, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand. f) Planer Mill (Strapper); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities, including but not limited to clearing strapping jams at the Strapper, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand. g) Planer Mill (Tilt Hoist and Planer infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards while performing servicing and maintenance activities, including but not limited to cleaning in and around the Tilt Hoist and Planer infeed conveyors, in that the employer failed to provide employees with written energy control procedures in a language employees could read and understand.
