29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warnings such that the minimum approach distance was being maintained before the minimum approach distance required by paragraph (d)(1) of this section was reached. On or about May 24, 2017, the job site supervisor was distracted and failed to warn the boom lift operator when the basket was moved in close proximity to a 138 kv voltage line exposing employees to electrical hazards.
