29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer is failing to protect employees from caught-in and struck-by hazards associated with operating and maintaining machinery. This most recently occurred on April 25, 2017 at the facility located at 4200 S. 121st Plaza, Omaha, NE 68137. The employer has employees removing parts for cleaning, performing minor maintenance, and removing jams from printers, gluers, tippers, and die cutters without following established procedures and utilizing lock-out/tag-out devices.
