Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employees were exposed to: a) The hazard of not being able to prevent the intrusion of formation fluids into the wellbore due to the failure of Farrar Oil Field Service & Equipment Co., Inc.'s crew of workover rig #4 to have a circulation/kill line physically connected between casing annulus of the R. & h. - Travis #3 Well and to kill fluid supply tank(s) to allow fluid to be pumped into the well with the blowout preventers closed during well control operations. This occurred on or about Tuesday, May 29, 1990, at this well located in Section 6, TION, R9W, Jasper County, MS. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to maintain physical pipe connections between the wellhead and sud tank(s), as specified in Section 4.A.1, A.P.I. RP-53, Recommended Practice for Blowout Prevention Equipment systems for Drilling Wells, Second Edition, 5/24/84, and Section 8.2, A.P.I. RP-59, Recommended Practices for Well Control Operations, First Edition, 8/20/87.
