This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri when an employee was fatality injured while breaking up Tetrazene inside the vacuum receiver in . The following are examples of missing steps within the Tetrazene standard operating procedure: (a) Wedging steps were not provided for the high explosive mixers who break up Tetrazene, an explosive compound that is dried inside the vacuum receiver in .
