29 CFR 1926.800(g)(5)(ii): On jobsites where less than 25 employees worked underground at one time, the employer did not provide or arrange in advance for at least one 5-person rescue team: a) Culvert Install Site: On or about May 05, 2017 and at times prior, as the controlling employer, Knife River did not ensure that its sub-contractor, Coleman Construction Inc., protected their employees in that employees were exposed to acute illness, burns, impairment of ability to self-rescue, and incapacitation while removing soil, inspecting, flame cutting, tacking and welding inside of a 36 inch culvert without an effective method for quick removal in the event of an emergency.
