29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee. Location: Roof of South Commissary Project, 20850 Vandegrift Boulevard, MCB Camp Pendleton, CA. Retraining was not provided to employees after deficiencies were identified with the warning line systems which indicated the skill and knowledge was not maintained. Warning lines were not erected at the correct distance from the roof edge and height from the working surface, around all sides of the roof, and they did not create a path of access to the work area. Employees were not protected from falling 23 feet to the lower level while accessing the roof and performing roofing work activities.
