29 CFR 1926.652(e)(1)(ii): Support systems were not installed and removed in a manner that protected employees from cave-ins, structural collapses, or from being struck by members of the support system. On or about March 10, 2017 at 2910 North Druid Hills Roads, Atlanta, Georgia: an employee working in a unprotected trench was exposed to a cave in hazard. To set the trench box the employee had to venture into the trench to unhook the chains used to set the trench box.
