Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. a. On or about March 6, 2017, located in Plant #8, employees were exposed to machine hazards associated with ingoing nip points while testing a newly installed pneumatically actuated seal that was located between the dust collector bin and purge collector. The employer failed to implement energy control application steps as LOTO devices were not applied to the pneumatic ball valve [per the 1910.147(d)(4) requirements] prior to testing. As a result, the remaining applicable energy control elements involving dissipation of residual energy [(d)(5)(I)] and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
