29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Site: in the equipment room adjacent to the maintenance manager's office, 175 Pioneer Ave., Leominster, MA 12345 on or about 3/15/2017 authorized employees performed maintenance requiring lock out/ tag out on the vacuum pumps without using a written energy control procedure.
