29 CFR 1926.452(a)(4) Diagonal bracing was not installed in both directions across the entire outside face of all double-pole, tubular welded scaffolds: a) 150 Forest Road, Staten Island, NY 10304. Employee was exposed to fall hazards of approximately 13 feet, 6 inches from the walking/working platform to the ground below when diagonal bracing was not installed in both directions across the entire outside face of the tubular welded scaffold resulting in the scaffold being unstable. Occurred on 03/03/17
