29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) North side of entrance overhang: Employee without any fall protection approximately 18-20 feet above ground level, without any type of fall protection, was standing on top of an unsecured sheet of OSB resting on the forks of the Model B-66 Pettibone Telehandler while removing construction debris from the roof of the overhang, boom was extended and retracted while employee stood on the platform, on or about 02/24/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
