29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about February 21, 2017, the employer did not ensure that employees cleaning the Resina Ball Placement Machine, Model 44-115, S# 947237, were protected from moving and rotating components by performing the following sequence of actions prior to servicing the machine: 1) Machine or equipment shutdown 2) Machine or equipment isolation 3) Lockout or tagout device application 4) Verification of isolation. Employees were exposed to amputation hazards while performing this task. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).
