29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person: a) At the jobsite, on or about 2/15/17: Employees were working from two point suspended scaffolds where the corresponding lifelines were draped across rope that was connected to the cantilevered counterweight beams. This method was not within the design of the scaffold system and would likely result in failure to the scaffold if engaged.
