29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources: a) Plant Floor/Processing Line, Band saw: On or about February 16, 2017 and at times prior, the employer exposed employees to amputation hazards, in that the employer had not provided authorized employees locks for isolating and securing machine energy sources in accordance with the established procedures.
