29 CFR 1910.252(a)(2)(iv): Cutting or welding was permitted in areas before the areas were inspected by the individual responsible for authorizing cutting and welding operations On or about February 8, 2017 in the Pulp Mill at the Foul Condensate Tank, the employer allowed employees responsible for issuing Hot Work Permits (HWP) to write them without visiting and inspecting the welding area; thus exposing employees to potential explosion hazards.
