29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): At 3883 Delor Street, St. Louis, MO 63116, specifically Building No. 2, Work Center #1902, the employer did not ensure that suitable lockout devices were used on the Bystronic Press Brake, m/n Beyeler PR320 1 PC, to isolate gravity. Authorized employee(s) were exposed to a serious caught-in hazard. This violation was observed on 02/13/17.
