29 CFR 1910.147(f)(4): During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming shift: a) On November 2016 and times prior, employees worked on equipment that was locked and tagged out for equipment repairs. The off-going shift briefed the oncoming shift but did not change out lockout devices between the two shifts.
