29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: (a) Turner Construction Company, 222 Milwaukee Street, Denver, CO 80206: On and before February 1, 2017, the employer failed to get verification or documentation from a competent person that ladder access was provided to the concrete formwork supported scaffold that was more than 2 feet below the concrete working surface.
