29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): The employer is failing to protect employees from overhead struck-by hazards associated with falling debris. The most recent occurrence of this was evident during the inspection of the facility located at 1700 E. Front St. Fremont, NE, where the employer has employees performing grain handling tasks in and around the facility to include working below bin number 303 which has an aging condition that causes chucks of concrete to fall randomly without control approximately 140 feet to the ground below.
