29 CFR 1915.12(b)(1)(ii): The employer did not ensure that spaces and adjacent spaces that contain or have contained combustible or flammable liquids or gases were tested by a competent person to determine the concentration of flammable vapors and gases within the space prior to entry by an employee: Location: Naval Base Coronado Port Operations in the boatyard: Testing for flammable and combustible vapors was not performed in the bilge area, which was adjacent to the fuel tank, prior to employees using an impact drill to torque the generator's foundation bolts to the vessel. Employees were exposed to fire and/or explosion hazards while using spark-producing equipment in the presence of fuel vapors.
