29 CFR 1910.212(a)(3)(ii): Point of operation guards are not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about January 25, 2017, and at times prior there to, an employee operated the Wyson and Miles Co. sheer machine that had a guard that did not restrict access to the point of operation. The employee was exposed to caught in between, crushed by, and amputation hazards.
