29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer is failing to protect employees from a (fall hazard) struck-by and crushed-by hazards. This was most recently documented on or about January, 20, 2017 at the jobsite located at or near Lot 27 on North Baltimore Ave. in Kansas City, Missouri. The employer did not provide or ensure that a ladder or other means of egress was available for employees working in the excavation. An employee was working in the excavation which was 8 to 13 feet in depth and approximately 43 feet in length.
