29 CFR 1910.147(c)(4)(i): Procedures were not documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 01/12/17 in the tumbler area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the Tumbler while the tumbler basket was rotating and their hand was in proximity to the drive wheel. They did not use a procedure to control the hazardous energy. b) On or about 01/12/17 in the packing area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the conveyors inside the Chilling Tunnel while the conveyor was operating without using a procedure to control the hazardous energy. c) On or about 01/12/17 in the packing area; Production employees were exposed to an amputation hazard when they changed over the Ishita rotary filler by removing and installing the baskets without using a written procedure to control the two hazardous energy sources ( air and electricity). d) On or about 01/12/17 in the packing area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the baskets on the Ishita rotary filler without using a written procedure to control the two hazardous energy sources ( air and electricity). e) On or about 01/12/17 in the raw area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the inside portions of the raw conveyor without using a procedure to control the hazardous energy. f) On or about 01/12/17 in the tumbler area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the wear strips and sprockets of the scale incline conveyor without using a procedure to control the hazardous energy.
