29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be denenergized were open. The employer did not render such means inoperable, unless its design does not so permit, and then sure that such means are tagged to indicate that employees are at work: (a) Location - Main Street, Malden, MA: On or about 12/20/16, the employer failed to ensure that a switch employees were opening was de-energized and exposed employees to electrical burns.
